The vision of the Saint Luke's Foundation, is to achieve measurable, sustained improvements in health and well-being by targeting the root causes of problems.

We believe that systemic change is required to bring about coordination among service providers so that older people will be able to access and use the services they need to maintain their quality of life.

> SISTERS OF CHARITY FOUNDATION
The Sisters of Charity Foundation of Cleveland is now in its 10th year of service as a funder and supportive resource serving those most in need. As of 2006, we have come together with the Saint Ann Foundation as a single organization primarily focused on issues related to poverty, women, children and youth across Northeast Ohio. To fulfill its mission, the Sisters of Charity Foundation of Cleveland provides grant funding to non-profit organizations that match our concerns and priorities.
